How to obtain your waiver to prescribe buprenorphine

As of 2021, MD/DOs, PAs, and NPs are no longer required to take an 8 hour waiver training.  Those who desire to prescribe buprenorphine for up to 30 patients are still required to obtain a buprenorphine waiver through the Substance Abuse Mental Health Services Administration (SAMHSA).

To apply, complete the Notification of Intent (NOI) form located at this link:  https://buprenorphine.samhsa.gov/forms/select-practitioner-type.php

If you’ve taken the waiver training and wish to request your waiver certificate:
1.    Go to www.PCSSnow.org/mywaiver
2.    Click on the button on the right side of the screen that says, “Request Your Waiver Certificate’
3.    You should have a response within 24-48 hours
